CONSIGNES DE SÉCURITÉ IMPORTANTES POUR LA PROCÉDURE DE PHOTOPHÉRÈSE THERAKOS™

Indications

Le système de photophérèse THERAKOS™ CELLEX™ est indiqué chez les patients âgés de plus de 18 ans pour réaliser une photophérèse dans les cas suivants :

Lymphome cutané à cellules T (Cutaneous T Cell Lymphoma - CTCL)
Rejet de greffe d’organe solide (Solid Organ Transplant Rejection - SOT) (cœur, poumon)

Le système de photophérèse THERAKOS™ CELLEX™ est indiqué chez les patients âgés de plus de 3 ans pour la prise en charge des éléments suivants :

Maladie aiguë et chronique du greffon contre l’hôte (GvHa, GvHc)

Contre-indications

La photophérèse THERAKOS™ est contre-indiquée :

Chez les patients présentant des antécédents de maladies photosensibles.
Chez les patients qui ne tolèrent pas une perte de volume extracorporel ou dont le nombre de leucocytes est supérieur à 25 000/mm³.
Chez les patients souffrant de troubles de la coagulation ou qui ont subi une splénectomie par le passé.

Avertissements et précautions

Les traitements par photophérèse THERAKOS™ doivent toujours être réalisés dans des établissements où des équipements d’intervention médicale d’urgence sont disponibles. Des liquides de remplacement de volume ou des expanseurs de volume doivent être gardés à portée de main tout au long de la procédure.

N’exposez pas le dispositif à un environnement de résonance magnétique (RM). Le dispositif peut présenter un risque de blessure par projectile et des blessures thermiques et des brûlures peuvent survenir. Le dispositif peut générer des artefacts dans l’image RM ou ne pas fonctionner correctement.
Des événements thromboemboliques, y compris l’embolie pulmonaire et la thrombose veineuse profonde, ont été signalés lors du traitement de patients atteints de la maladie du greffon contre l’hôte (GvHD). Il est conseillé de faire particulièrement attention à une anticoagulation adaptée lors du traitement de patients atteints de GvHD.
Lors de la prescription et de l’administration de la photophérèse THERAKOS à des patients recevant un traitement concomitant, vous devez faire preuve de prudence lorsque vous modifiez les plans de traitement afin d’éviter une recrudescence de l’activité pathologique susceptible d’être provoquée par l’arrêt brutal du traitement précédent.

Événements indésirables

Une hypotension peut survenir pendant tout traitement impliquant la circulation extracorporelle. Surveiller le patient de près pendant tout le traitement afin d’identifier toute hypotension éventuelle.
Des réactions pyrétiques transitoires (37,7 °C–38,9 °C [100–102°F]) ont été observées chez certains patients dans les six à huit heures suivant la réinjection du sang enrichi en leucocytes photoactivés. Une augmentation temporaire de l’érythrodermie peut accompagner la réaction pyrétique.
Une fréquence de traitement dépassant les recommandations indiquées sur les étiquettes peut entraîner une anémie.
L’accès veineux implique un léger risque d’infection et de douleur.
